PCOS /PCOD/ INFERTILITY
PCOS-Polycystic ovary syndrome (PCOS) is a common endocrine and metabolic disorder in reproductive-aged women, characterized by a combination of high androgen levels (hormonal imbalance), irregular or absent menstrual periods, and/or small fluid-filled follicles on the ovaries. It is a leading cause of infertility and can lead to long-term health issues like insulin resistance, Diabetes & Endometrial Cancer.
Key Components and Features of PCOS:Hormonal Imbalance (Hyperandrogenism): Often characterized by high levels of androgen ("male" hormones like testosterone), leading to symptoms like acne, hirsutism (excess facial/body hair), and thinning scalp hair.Ovarian Dysfunction: Irregular ovulation or anovulation (no ovulation), which causes irregular or missed periods."Polycystic" Ovaries: The presence of many small, immature follicles (sacs) on the ovaries, which are often mistaken for cysts. These are actually undeveloped follicles, not painful cysts.Metabolic Issues: Up to 85% of women with PCOS have insulin resistance, which can lead to weight gain, metabolic syndrome, and a higher risk of Type 2 diabetes.
Causes-
Factors that might play a role include:
Insulin resistance-Insulin is a hormone that the pancreas makes. It allows cells to use sugar, your body's primary energy supply. If cells become resistant to the action of insulin, then blood sugar levels can go up. This can cause your body to make more insulin to try to bring down the blood sugar level. Too much insulin might cause your body to make too much of the male hormone androgen. You could have trouble with ovulation, the process where eggs are released from the ovary. One sign of insulin resistance is dark, velvety patches of skin on the lower part of the neck, armpits, groin or under the breasts. A bigger appetite and weight gain may be other signs. Low-grade inflammation.- White blood cells make substances in response to infection or injury. This response is called low-grade inflammation. Research shows that people with PCOS have a type of long-term, low-grade inflammation that leads polycystic ovaries to produce androgens. This can lead to heart and blood vessel problems. Heredity-Research suggests that certain genes might be linked to PCOS. Having a family history of PCOS may play a role in developing the condition. Excess androgen -With PCOS, the ovaries may produce high levels of androgen. Having too much androgen interferes with ovulation. This means that eggs don't develop on a regular basis and aren't released from the follicles where they develop. Excess androgen also can result in hirsutism and acne.
Complications
Complications of PCOS can include:-
Infertility
Gestational diabetes or pregnancy-induced high blood pressure Miscarriage or premature birth Nonalcoholic steatohepatitis — a severe liver inflammation caused by fat buildup in the liver Metabolic syndrome — a cluster of conditions including high blood pressure, high blood sugar, and unhealthy cholesterol or triglyceride levels that significantly increase your risk of heart and blood vessel (cardiovascular) disease Type 2 diabetes or prediabetes Sleep apnea Depression, anxiety and eating disorders Cancer of the uterine lining (endometrial cancer).
Symptoms of PCOS-
Irregular periods: Women with PCOS may have no periods, irregular periods or very long periods. Irregular periods may be a sign the ovaries are not releasing an egg each month.
Acne:- Adult acne can be a sign reproductive hormones are out of balance.
Hair growth-Hair grows in places that should not have hair – like the chin, back or chest. This kind of hair growth could mean too many male hormones are being produced.
Multiple cysts-: Women with PCOS have a lot of cysts on their ovaries. Cysts normally are found with ultrasound, which uses a small transducer (probe) and ultrasound gel placed on the skin.
Hair loss:- Male-pattern baldness or hair loss may indicate women have a higher level of male reproductive hormones, which is a sign of PCOS.
Weight gain:-Women with PCOS often can't lose weight. They have a higher chance of insulin resistance, where the body does not respond to insulin, a hormone that keeps blood sugar in normal range. This may require changes in diet and activity or medications to assist with weight loss.
Infertility-: PCOS is a common cause of infertility in women. Women with PCOS may have trouble becoming pregnant because they ovulate irregularly or not at all. For this reason, timing intercourse with ovulation predictor kits may not work. Instead, many women with PCOS benefit from fertility treatments like IVF.
Tiredness:- Women with PCOS have a higher chance of having sleep apnea, a condition that causes breathing to stop and start repeatedly during the night. Despite going to bed on time and trying to get 8 hours of sleep, they wake up tired, causing them to feel fatigued all day.
Mood swings- Because of the hormonal imbalance, women with PCOS also have a higher risk for depression, anxiety and extreme or rapid changes in mood.
Heavy, painful periods+: PCOS can cause painful periods with heavy bleeding. This can cause additional issues, such as anemia, and is often unpleasant and even scary.
Types of PCOS-
Insulin Resistant PCOS: -The most common type (up to 70% of cases), caused by high insulin levels that inhibit ovulation and increase testosterone. Often associated with weight gain.
Inflammatory PCOS:- Driven by chronic inflammation, leading the body to produce excess testosterone, resulting in irregular cycles, acne, and fatigue.
Post-Pill PCOS:- Occurs after stopping hormonal birth control, causing a temporary, often temporary, surge in androgens.
Adrenal PCOS:- Triggered by high stress, causing the adrenal glands to produce excess androgen (DHEAS) rather than the ovaries.

PCOD = Disease (ovary) Local Ovarian Problem
PCOS = Syndrome (systematic) Whole Body Hormonal Problem
PCOD (Polycystic Ovarian Disease)
The problem lies inside the ovaries.
Caused by Lifestyle factors (Poor Diet / Stress / Obesity/ Weight Gain / Lack of Exercise)
Temporary Hormonal Imbalance
o Between LH & FSH Egg doesn't mature properly.
o Slightly increase Androgens (Male Hormones)
Result Ovaries produce immature eggs in Cysts Form seen in ultrasound.

INFERTILITY
Anovulation Excess androgens prevent the follicles (sacs containing eggs) from developing and releasing a mature egg each month.
Insulin Resistance High insulin levels can trigger the ovaries to produce even more
androgens, further disrupting the ovulation cycle.
Egg Quality & Implantation Hormonal imbalances can sometimes affect the quality of eggs or lead to an unstable uterine lining, making it harder for a fertilised egg to implant.
Treatment Diet + Exercise + PCO–BEST Tablet = Freedom from INFERTILITY.
